when asked for a copy of my mentally challenged sons physical to be sent to department on disability services the report was never sent, now i have to go in person to make sure they get a copy ,i am disabled to so it is hard that they did not comply with my request(one of the staff who weighs the clients) i provided the fax number and dds still did not get the report neithe did socical security.
