Columbus Health and Rehabilitation Center accommodates 235 residents. We offer a continuum of healthcare services. Our center also provides a Special Care Alzheimer’s Unit. We are licensed by the State of Indiana and are a Medicare and Medicaid approved provider. We are proud to offer a friendly home-like atmosphere while providing comprehensive healthcare services. These services include short-term medical and rehabilitation treatment, which is designed to address the individual needs of our residents and patients. In addition, we offer long-term intermediate and skilled nursing services. Columbus Health is a 2004 recipient of the American Health Care Association’s Quality Award.

Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential.
