About Our Nursing and Rehabilitation Center Bridging the Gap Between Hospital and Home Our nursing and rehabilitation center is conveniently located north of Kuebler Boulevard in South Salem, near the I-5 Exit. We offers short-term rehabilitative as well as long-term care - a full range of medical, nursing, rehabilitation and social services to treat and support each of our patients and residents It is the goal of our rehabilitation group to maximize independence and functioning in daily life. Our occupational therapists use a complete kitchen, equipped with dishwasher and microwave, for therapy. This provides a genuine home-like setting, where mobility and function can be monitored by trained professionals and daily living skills can be tailored to fit individual needs. We are available 24-hours a day, seven days a week for admissions. Our Admissions Department works closely with all hospitals including Santiam Hospital, Salem Hospital and Silverton Hospital. Patients and residents have come from other hospitals throughout the state as well. Our center also has a working relationship with Hospice of the Northwest, Serenity Hospice, Signature Hospice and Willamette Valley Hospice. Our center earned the American Health Care Association’s Bronze – Commitment to Quality award. It is our goal to provide safe, compassionate care in an environment that fosters independence and dignity.
