Lutheran Home at Napoleon, a ministry of Lutheran Homes Society, offers a full continuum of care for retirement living. The Napoleon campus features independent living at Bavarian Village, assisted living at Alpine Village, and medical care and therapy services at the Lutheran Nursing and Rehabilitation Center. Our medical care services include long- and short-term nursing care, dementia care, complex medical care, chronic disease management, palliative and hospice care, and respite care.

Services At the Lutheran Nursing and Rehabilitation Center, a wide range of medical care services is provided by a staff dedicated to person-centered, compassionate care. Person-centered care focuses on the needs and wants of each resident, and allows residents to have more choices in their daily routine, care, and activities. The care center has a medical director, partnering physicians, a nurse practitioner, and 24-hour staffing that includes registered nurses. We provide numerous medical care services, including: Skilled Nursing Care Long- or short-term residential nursing care is provided 24 hours a day, seven days a week by a staff of registered nurses, licensed practical nurses and nurse aides. It’s the caring staff who makes this a home for residents. The care center features an excellent staff-to-resident ratio for an optimal level of care. Short-stay Transitional Care When individuals require short-term placement following a hospital stay, our experienced nursing care staff teams up with our therapy department to provide the care needed to return home within the time allotted by insurance providers. Our clinical team collaborates with patients’ medical doctors to develop a specialized plan of care to meet all of the patients’ health care needs. We offer on-site case management services and home assessments prior to discharge to home. Complex Medical Care The Lutheran Nursing and Rehabilitation Center specializes in wound care services. These services include a wound care certified nurse practitioner who visits bi-weekly and as needed. The skilled nursing staff is experienced in wound VACs, pin care, surgical wound care, pressure ulcerations, and skin tears and abrasions. We also support many other complex medical needs, including: •Stroke recovery •Diabetes management •IV Therapy (including most types of central lines) •Tracheostomy care •Enteral/parenteral nutrition and other complex clinical nutritional concerns •Cardiac health management •Pulmonology and complex respiratory care management •Dialysis care Chronic Disease Management For people suffering from a long-lasting condition that can be controlled but not cured, the care center can provide some relief. Not only is our staff trained to work with patients with chronic illnesses, but we also develop and manage a Chronic Disease Care Plan in conjunction with medical physicians and other clinical specialists to prevent exacerbation of the disease process and re-hospitalization. Dementia Care The Lutheran Nursing and Rehabilitation Center can also provide care for individuals with moderate to severe memory loss. Our specialized dementia care includes safety monitoring with the WanderGuard® wander management system. Monitors “lock down” the facility’s doors as needed for resident safety. Palliative and Hospice Care When end-of-life care becomes necessary, the Lutheran Nursing and Rehabilitation Center works with area hospice providers to supply the medical services, emotional support, and spiritual resources required by people who are in the last stages of a serious illness. Palliative and hospice care focuses on relieving pain and other symptoms to enhance the quality of remaining life by keeping patients as alert and comfortable as possible in a familiar environment with family and friends. Hospice care also helps family members manage the practical details and emotional challenges of caring for a dying loved one. Respite Care Many people are providing care to loved ones in their own homes. To give caregivers a chance for a much needed vacation or break, we can provide short-term, temporary stays for individuals. Respite stays, which can vary in length from a few days to several weeks, provide peace of mind for caregivers as loved ones receive care in a nurturing, professional environment.
