My daughter developed nocturnal generalized tonic clonic seizures at age 19 in college. She saw this doctor when she came home a few short weeks later. She complained that her medication made her feel “sleepy all the time” and that she didn’t think she needed to take the medication even though she was continuing to have seizures. He ignored these “red flags” and did not address her concerns with education about SUDEP and the potential life-and-death consequences of noncompliance even though she was at high risk due to multiple risk factors, three of which could have been eliminated altogether if we had only known!! She was very active, playing on 4 soccer teams at the time of her shocking and completely unexpected sudden death 11 days after her 27th birthday. The average age of death is 26! She needed continuous nighttime monitoring with a roommate, seizure alert dog or movement monitor but we were never even aware that she could die in her sleep. At that time she could not have been in a more unsafe situation as her room was in the downstairs apartment of a townhouse two floors below her 3 housemates. She cruelly died alone in this apartment since no one could hear her in distress and come to her aid. She might only have needed to be turned over since she died facedown in bed as ~70% of SUDEP victims do. She was found dead in her bed the next day after a welfare check was requested by her employer after she did not show up for work and was not answering her phone. Regardless of risk level ALL patients at any risk of SUDEP and caregivers MUST know this to prevent these senseless deaths. Cochrane medical guidelines for the treatment of epilepsy have recommended this education since 2017 but ~90+% never discuss the risk with their patients and the few that do are not consistent in doing so. Research is showing that -~80% of these deaths are AVOIDABLE!! I am convinced that if someone had been with her or could have only heard her in distress, she would be alive today. It was immoral and deplorable that we did not know about SUDEP. My daughter was an accomplished academic and athlete who had been skipping doses of her medication only because she was playing on 4 soccer teams and the side effects affected her energy level. She would NEVER have risked her LIFE by not taking her medication consistently had she been properly educated about her condition!!. She had a very bright future and was planning on applying to law school. She had worked so hard and achieved much in her 27 years but was essentially a “sacrificial lamb” since not one of her doctors cared enough about whether she lived or died to educate her fully about her condition and, especially, SUDEP. Her death was so shocking and catastrophic we were completely destroyed as a family along with her many friends and colleagues. What happened to her was shameful and immoral and should never happen to anyone but thousands of, mostly, young adults and children die every single year simply because doctors don’t want to discuss SUDEP with their patients. Common excuses are that the patient might become depressed or anxious, both of which can be treated. DEATH CANNOT!! The problem is so widespread that in 2022, NY became the first state in the country to mandate SUDEP education to all patients at risk. A father who also lost his only daughter at a similar age was able to get the legislation passed which is what I am trying to do in my state. It is pathetic that doctors would have to be ordered to do what they should be doing routinely but this is absolutely the case.
