I was assured that my insurance provider would cover all but about $100. A month later they sent me a bill for $500.

RIP OFF!!!!

It could have used some cleaning
Was this provider willing to stay open late to accomodate your needs?
No, they close exactly on schedule despite any circumstances
Did your mouth feel clean after your appointment?
Not really, it wasn't as clean as I hoped it would be
